Lhutel the Headless	Boss Drop: Tombsward Catacombs Defeat the Cemetery Shade at the end of the catacombs.
Redmane Knight Ogha	Boss Drop: War-Dead Catacombs Defeat the Putrid Tree Spirit at the end of the catacombs.
Mimic Tear	Loot: Nokron, Eternal City Found in a chest locked behind an imp statue door in Night's Sacred Ground.
Black Knife Tiche	Boss Drop: Moonlight Altar Defeat Black Knife Ringleader, Alecto in Ringleader's Evergaol.
Ancient Dragon Knight Kristoff	Boss Drop: Sainted Hero's Grave Defeat the Ancient Hero of Zamor at the end of the hero's grave.
Cleanrot Knight Finlay	Loot: Elphael, Brace of the Haligtree Found in a treasure chest guarded by a Cleanrot Knight and a Haligtree Soldier. The room's entrance is on a rooftop with a folly nearby and can be accessed by jumping off a flying buttress north of the Prayer Room Site of Grace.

Unlike the Legendary Armaments and Legendary Talismans, this achievement/trophy cannot be obtained through online trading.
